3歳未満の幼児に対する純音聴力測定法—条件詮索反射聴力測定（COR-audiometry）について

　I．緒言

　3歳以上の幼児の純音聴力はDix and Hallpike^1）のPeep-show testの様ないわゆるPlay audiometryの導入により成人に於ける自覚法の場合とそれ程違わない精密さで測定されるようになり，検査法が簡便なため臨床的に広く用いられつつあるが，3歳未満の乳幼児に対してはこのような精密で，しかも実施の容易な方法は工夫されておらない。勿論GSR-audiometry，脳波聴力測定法等がこの目的の為に検討されてはいるが，これ等は日常臨床的の方法として誰にでも手軽に実施出来ると云う性質の方法ではなく，更に方法自体にも種々の解決すべき問題が残つている。要するに3歳末満の幼児に対しては臨床的な純音聴力測定法は見当らないというのが現状である。

　私は幼児に興味ある視覚刺激を与えると幼児はその方を振り向くと云う反応（詮索反射）を利用し，以下述べる様な装置で主として3歳以下の幼児の純音聴力測定を試みた所，臨床的方法として充分使用し得る方法であるとの確信を得るに至つた。1960年1月鈴木及び著者^2）はこの新しい聴力測定法をRevue de Laryngologie誌上に，"A technique of pure tone audiometry for children under three years of age. Conditioned orientation reflex (C. O. R.) audiometry"と題してはじめて発表したが，ここでは主として測定装置及び測定方法を紹介し，諸賢の御追試，御批判を仰ぐ次第である。

　Making use of the sense of orientation and refex in hearing tests in children below age 3 the author reports a new pure tone hearing test in which the child's attention is called by interesting sound or that he had never heard before. This method of testing is named conditioned orientation reflex audiometryor coraudiometry. The test is realized by exercise of two factors, the orientation and reflex as unconditional response on one hand and sound stimulation as a conditional stimulation on the other. Children from the age of 1 to 3 are most suited for this test. Equipments needed for the test and the method are fully described.
