上顎洞経由篩骨蜂窩開放手術中蜘蛛膜下腔出血を起した1症例

　I．緒言

　鼻性頭蓋内合併症としては，現在までの報告の総てが鼻性化膿性脳膜炎であり，その過半数が手術或いは何等かの操作に起因している。然し術中蜘蛛膜下腔出血を惹起したと云う報告はない。私はかつて上顎洞経由節骨蜂窠開放手術中に蜘蛛膜下腔出血を起したが，幸い治癒した例を経験したので茲に報告し，併せて篩骨蜂窠天蓋及び篩板の解剖学的関係，並びに本邦に於ける鼻性脳膜炎についての発表と比較検討し，考察を加えてみたい。

　Satoh reports that he accidently caused a subarachnoid hemorrhage in a woman, aged 22, during the procedure of establishing local anesthesia in the ethmoids for an operation on these structures that followed that of the maxillary sinus. The needle employed for the injection was 6.5cm in length which apparently entered the subarachnoid space without any appreciable resistance whatsoever. With the aid of antibiotics and spinal punctures the patient recovered from the meningitis in about 40 days.
