 Because subglottic cancer is highly malignant in that it is chracterized by a epecific mode with which it progresses clinically and a tendency of recurrence the diagnosis of this condition in its early stages is highly important and desirable. For this purpose an approach is chosen whereby the trachea may be opened through the interspace of between the cricoid cartilage and the first tracheal ring, a procedure that could be carried out with the patient under local anesthesia. The subglottic region would be exposed to a direct inspection in which biopsy specimen may be removed for a frozen section diagnosis. by pathologist while the surgeon is waiting for the latter's report. If the report is positive for malignancy the patient may be changed over to intratracheal anesthesia at this point for total laryngectomy.

 The authors report 4 cases of subglottic malignancy that required total laryngectomy. The laboratory report in all 4 cases from examination of biopsy specimens obtained per oral route proved to be negative; verification of positive diagnosis of malignancy was made by employment of other route of approach. In one case among them the above method was used.
