外耳道に原発せるMelanom及びOsteomに就いて

　外耳の発生は第一鰓弓の下顎突起と第3鰓弓の増大によつて，第1鰓溝が漸次深みを形成し，之が外耳道の原基となる。前者の縁，即ち第1鰓弓の下顎突起と第2鰓弓とに小隆起の形成が起り，之が耳介の基礎を造る。これらが漸次成育するに従い耳介と外耳道は形態的にも，組織的にも相当の差異を生じ，そのため同一原因の疾患に侵されても症状，経過等に差異を見るのは日常の臨床に於いて経験する所である。尚耳介，外耳道共に皮膚一般に生じ得る良性，悪性の各種腫瘍が発生するが，その頻度は，身体の他部に比して比較的尠い。殊に外耳道に於いては耳介よりも更に稀である。外耳道に生ずる良性腫瘍としては乳頭腫，骨腫，内皮細胞腫，軟骨腫，母斑，黒色腫，プラズマ細胞腫，神経腫，脂腺腫，皮膚嚢腫，嚢腺腫，等が知られてはいるが，その報告は内外を問わず極めて稀で，各々数例に過ぎない。外耳道め原発癌にしても本邦文献中に約10例の報告を見るのみであるが，外耳道の原発肉腫に到つては僅かにKuhne（1869）のMelanosarcomaの一例を見るのみである。我々は斯くの如く甚だ稀とされている外耳道の原発性Melanomを2例，Osteomを1例経験したので，従前の症例の追加として報告する次第である。

　Imakiire and associate rcport two, cases of primary melanoma and a case of osteoma that occurred in each instance in the external auditery canal, The melanoma was fonnd in the first case in a man, aged 23, and in the second, in a woman, aged 60, during the course of geueral ENT examination; no subjective complaints wore made by the patient in either case because of its presence. Both cases made unevedtful recovery after aurgical remsval of the growth. Histologically the growths consisted of nevus cells haboring typihal cancer nests in the corinm and the overall histological picture conformed to that ot precancerous state of melanotic growth.

　The third case is a woman, aged 25, who wds found to be affected with a hard growth in the left external auditory canal. The tumor was romoved surgically by rcfroauricular approach with the patient making an uneveneful recoyery. The growth proved to be an ostema that originated from the perioteum of the posterior wall of the external anditory canal.
