外耳皮膚ヂフテリーに就いて

　緒言

　粘膜系統のヂフテリーに比して皮膚ヂフテリーは稀有である．文献に就いて皮膚ヂフテリーを拾つて見ると，耳，眼瞼，頬部，臀部，背部，四肢等何所の皮膚にも見られて居る．而して左程珍らしいものでもないけれども，最近著者は不注意にも診断の確定が遅れた爲めに，計らずも耳翼ヂフテリーの1症例に就いて，其の初期症状から治癒に至る迄の全経過を観察する機会を得たので，其の臨床的な所見を述べて参考に供し度いと思ふ．大体皮膚ヂフテリーは一般症状の軽いものが多いので，医師の許を訪ねるのが遅れる傾向がある．從局多くは其の初期症状を見られず，潰瘍又は濕疹の状態を呈してから見られる．此の意味に於いて其の初期から治癒迄観察し得た本症例に聊か興味を感じた次第である．以上の樣に本症が医診を受くる事が遅れ勝ちであり、又時としては患者は早期に來たが医師の診断確定が遅れたりするので，他に傳染させる機会が多い事も考えられ，傳染予防の上にも警戒を要するのである．其の良き実例として，古い実驗例であるが第2例を併せて報告し度いと思ふ．

The pinna above the external acoustic meatus in the beginning appeared with rosy pink disco-loration which lates changed to minute localized ulcerations and necrotic areas with formation of pseudomembranes. Persistency of dermal lesi-ons which lasted tor 32 days, in one case, wi-thout remission in spite of treatments with va-rious therapeutic agents Iead to suspect dipht-heria as the likely underlying cause of the dise-ase, Smear examination with methylene blue stain revealed rou-shaped becilli which were thought to be diphtheritic in character. The au-thor states relatively large amount of diphtheria anti-toxin was required to establish the cure ; 7500 units were given in divided doses of 1500 units at intervals, two days apart. Complete remission of the disease occurred on the 48th day since the beginning of anti-toxin injections.
