氣管套管抜去困難症の3治験例

　緒言

　救急処置として気管切開が行われ，其の際挿入された気管套管が，其の原因となつた疾病が去つた後に於ても引続き気管套管の拔去不能の状態に陥る場合は決して稀なものではない．従来斯る症例の報告は多数見られる処であつて，其の原因としては大別して見ると，気管套管を抜去することにより招来される狭窄感に基因する精神的要素に基づく場合と，続発性に起つた気管疾患に因る狭窄の為に套管抜去不能を誘致する如き器質的に呼吸困難の原因の存在する場合と，此の2つの場合が挙げられている．従つて其の治療に当つても此の2つの原因が色々な形に於て拔去不能の原因を構成している為に，一樣一律に処置する事は困難であつて，従来幾多の器械が考案せられている処であり又個々の症例に応じて種々の手段工夫が施されている所以である．余は原因及び治療に於て夫々異つた興味ある症例を得たるを以て茲に従来の文献に追加報告する次第である．

NAGOSHI discusses the difficulties whih which the proceedure of decanulization may be encountered in post-tracheotomy cases in whom the original operation was necessiated by reason of suicidal atte-mpts, foreign bodies in the trachea or laryngeal diphthera. He states that, particularly in children, edema of tracheal mucous membranes in general as well the area immediately below the vocal co-rds contribute important factors whereby acute dy-spnea and laryngeal stridulus which are almost devoid of treatment are established. But, there are cases when even under such circumstances the pa-tient may recover himself by a natural prccess of healing.
