I．はじめに

　脳血管撮影技術の進歩とともに動脈解離が正確に診断されるようになり，本疾患は若年者の脳血管障害の原因として注目されるようになった．しかし，その自然歴については不明な点が多く，治療法も一定していない．われわれは後頭蓋窩の虚血発作を繰り返し，脳血管撮影で自然寛解を示した頭蓋外椎骨動脈解離の1例を経験したので若干の文献的考察を加えて報告する．

Spontaneous dissection of the vertebral artery is not a frequent occurrence in the vertebrobasilar system. The authors show a case with spontaneous dissection of the extracranial vertebral artery, which produced recurrent artery-to-artery embolism. The angiography revealed characteristic intimal flap. The antiplatelet and anti-coagulant therapy was able to control ischemic attacks and the patient made a good clinical recovery. Theangiogram performed two months later showed spon-taneous resolution of the artery involved. This case shows the effectiveness of anticoagulation and anti-platelet therapy for the prevention of artery-to-artery embolism caused by the extracranial vertebral artery dissection.
