著明な走行異常を示した前大脳動脈に認めた前頭眼窩動脈瘤の1例

An Aneurysm of the Distal Anterior Cerebral Artery with a Remarkably Anomalous Configuration

I．はじめに

　前交通動脈より遠位部に発生する動脈瘤は一般に末梢性前大脳動脈瘤と呼ばれており，全脳動脈瘤の約5％を占める比較的稀なものである^{4-6,8,11,12）}．この動脈瘤は前大脳動脈の走行異常を高頻度に伴っていること^4-6,8,11），主幹動脈が大きく屈曲する先端部に出現することが多く，その発生にhemody—namicsの因子が大きく関与していること^4,5），比較的大きなものが多く，動脈瘤内に血栓形成を伴うものが多いこと^5）などが特徴とされている．

　このたびわれわれは走行異常を伴った前大脳動脈の前頭眼窩動脈部分に発生した動脈瘤を経験した．同部位の動脈瘤症例はいまだ1例の報告をみるのみであり^2），非常に稀なものと考えられるために，その脳血管撮影所見，手術所見を中心に報告する．

A case of aneurysm on the distal anterior cerebral artery with a remarkably anomalous configuration was reported. In the present case, the left anterior cerebral artery showed hairpin-like angulation, at the top of which a small saccular aneurysm was noticed. This anomalous anterior cerebral artery seemed to feed the territories of the frontoorbital artery and the pericallo-sal artery. Although the incidence of distal anterior cere-bral artery aneurysm is low, they are very interesting due to several characteristics.
