前大脳動脈瘤の手術

Surgical Treatment of Distal Anterior Cerebral Artery Aneurysms

Ⅰ．はじめに

　前交通動脈分岐部より末梢の前大脳動脈に発生した動脈瘤（distal anterior cerebral artery aneurysm）は，発生頻度が少なく^8）1人の術者の経験する症例数^{1，4，5，7，15，16）}が乏しいことなどにより，この部の動脈瘤の発生部位による特殊性についての追求はい虫だ十分にはなされていない．我々は1973年12月31日までに，A₂以下末梢に発生した前大脳動脈瘤を46症例経験し，45症例に直接手術を施行した．これらの症例をもとに，この部の動脈瘤に関する手術術式について一定見が得られたので報告する．

Forty-five patients with aneurysms of the anterior cerebral artery distal to the anterior communicating artery were operated on by a direct approach method in the years 1960-1973. The incidence of aneurysms in this location was 4.8% of the total 1,000 aneurysms.

It is of upmost importance in the treatment of aneurysms to insure the parent artery for the purpose of temporary occlusion. This makes it easier and safer to approach the aneurysmal neck and to handle possible premature aneurysmal rupture.
