Cerebral arterial ectasiaの4例—その臨床像と治療法について

Four Cases of Cerebral Arterial Ectasia: Its clinical manifestations and surgical treatment

I．はじめに

　異常に拡張，蛇行した脳血管はさまざまな名称で呼ばれており，たとえばarteriosclerotic aneurysm^5），fusi—form aneurysm^1,11,13），aneurysmal malformation^16），ser—pentine artery^15），megadolichoectasia^14,17,20），arterialectasia^2,19）などの名称が使われている．これらの血管は臨床的には種々の脳神経の圧迫症状を呈したり，第3脳室を圧迫して水頭症をきたして発見されることが多いが，ときにはくも膜下出血をきたすことがあり，また虚血性脳病変を生じることも報告されている．われわれは最近，4例のcerebral arterial ectasia症例を経験したので，その臨床像と脳血管撮影所見を報告し，その治療法について考察を加えた．

Four cases of intracranial arterial ectasia are reported. The clinical manifestations of the four cases due to the arterial ectasia were ischemic attack in two patients, subarachnoid hemorrhage in one patient and abducens palsy in one patient. Surgical treatment was performed in three patients.

The angiographic findings of the four patients were reported and the pathogenesis and the possibility of the surgical treatment of the arterial ectasia were discussed.
