I．はじめに

　鼻腔・副鼻腔原発の悪性腫瘍が，頭蓋底部を浸潤・破壊し，頭蓋内に進展して海綿静脈洞症候群をはじめとする種々の神経症状を呈することはよく知られている．頭蓋底部を形成する副鼻腕には，解剖学的にlocus minor—isが存在し，mucoceleなどの嚢胞性病変，良性腫瘍を含めた腫瘍性病変の，格好の頭蓋内進展経路となっている．こうした頭蓋底部腫瘍の発生・進展様式を明らかにすることは，手術アプローチをはじめとする治療法の選択に極めて重要である．われわれは発症より10年の経過で再発をくり返しつつ広範に頭蓋底を破壊し，頭蓋内に進展した良性歯原性腫瘍の極めて稀な1例を経験し，手術所見および神経放射線学的所見から，腫瘍の進展様式を考察したので報告する．

A 49-year-old female of adenomatoicl odontogenic tumor with intracranial extension was reported.

She had 3 months history of left nasal obstruction and visual disturbance. Endocrinological examinations revealed a low basal level of serum cortisol and low level of urinary 17-OHCS. Neurological examinations revealed left anosmia and left visual disturbance (hand movement, marked afferent pupillary defect, and lower hemianopsia).
