I．はじめに

　von Hippel-Lindau病は，網膜・小脳の他に脊髄や腹部臓器にも血管芽腫などの病変を有し，多彩な症状を呈することが知られている^6,7,11）が，本病における脊髄症状の出現頻度は4-6％程度とされ，眼症状や小脳症状に比べて稀である^6,7）．　今回われわれは，脊髄症状のみで発症したvon Hip—pel-Lindau病の1症例を経験した．本症例は脊髄に多発血管芽腫と脊髄空洞症を有しており，両者の因果関係や診断上の問題点などについて若干の考察を加えたので報告する．

A rare case of von Hippel-Lindau disease showing only spinal cord symptoms is reported. This 18-year-old boy was admitted to our clinic because of urinary disturbance, slowly progressive muscle weak-ness and sensory disturbance of his body and four extremities. Myelography demonstrated an expanding spinal cord and a figure of complete block of contrast medium at the lower cervical level. Dilated syrinx below the level of Th-3 was shown by delayed scan (19 hr.) of metrizamide CT myelography. These findings suggest that metrizamide could pass through the cord substance from the suharachnoid space into a syrinx cavity.
