全身骨にMultiple metastasisをきたしたAnaplastic oligodendrogliomaの1例

Diffuse Bone Marrow Metastasis by Anaplastic Oligodendroglioma

I．はじめに

　脳腫瘍の頭蓋外転移は稀な病態であるが，今回われわれが経験したoligodendrogliomaの頭蓋外転移は特に稀で，調べ得た範囲では文献上わずかに12例が報告されているのみである^14）．oligodendrogliomaを含むglio-ma　の頭蓋外転移の原因としては，開頭手術との関連を指摘する説が有力であるが，われわれの症例も2回の開頭手術を行っており，発症より約6年後に急速に全身骨転移が進行し，死亡した．転移の原因としては，ほかに近年治療の進歩に伴って，長期生存例が増加したためと考える説もある^1,9）が，もしそうなら，今後gliomaの頭蓋外転移はさらに増加する可能性があると考えられる．そこで文献的に，頭蓋外転移の頻度，原因，診断について検討するとともに，治療法についても考察を加えた．

The authors report a case of anaplastic oligoden-droglioma, in which multiple bone metastases develop-ed about 6 years after the first craniotomy.

A 32-year-old woman was admitted because of an apathetic state. CT scan and angiography suggested right frontal glioma. On August 15, 1977, a right frontal lobectomy was performed and histological sections showed anaplastic oligodendroglioma. Post-operative course was uneventful and she was discharged after chemoradiotherapy.
