頑固な頭痛を主訴とするPrimary empty sella症例に対するTranssphenoidal surgery

Treatment of Primary Empty Sella with Intractable Headache via the Transspehnoidal Approach

I．はじめに

　いわゆるprimary empty sellaは，1951年Buschの報告以来，剖検データでは5.5-23.5％と稀なものではないと考えられている^3,6,13）．臨床的にはほとんどの例が無症候性であり，それ自体無害な，いわば一種のana-tomical variationともいうべき状態であるが^13,16），ときに視野，視力障害あるいは髄液鼻漏などをきたすことがある^7）．また多くの例で頭痛を伴うとされているが，これは通常前額部から眼窩部における頑固な持続性の疼痛である^7,16）．そして，これらの頭痛は特別な治療の対象となっていないのが現状である^7）．

　今回われわれは頑固な頭痛を主訴とする2例のpri-mary epmty sella症例に対して，transsphenoidal ap-proachにて手術治療を行い，頭痛の消失をみたので，症例を報告するとともに，頭痛を主訴とするprimaryempty sellaの手術療法の可能性について考察する．

Two cases of primary empty sella with intractable headache were treated via the transsphenoidal approach. One patient was a fifty-three-year-old female with right upper nasal quadrantanopsia and intractable retrobulbar pain and the other was fourty-six-year-old female with continuous retrobulbar pain with a history of transient right temporal hemianopsia. Both cases were diagnosed by metrizamide CT cisternography. They had normal endocrinological functions. They did not respond to drug therapy and were treated surgically.
