I．はじめに

　primary empty sellaとはトルコ鞍横隔膜の欠損により，くも膜下腔が嚢腫状に拡大してトルコ鞍内容の大部分を占め，その結果，下垂体が圧排され，鞍内が一見空虚になっている状態をいい，剖検例の5.1％に認められる^3）．頭蓋単純写上その約90％はトルコ鞍拡大を伴い^1,18），逆にトルコ鞍拡大例の8-25％^9）にempty sellaが認められるという．

　したがって下垂体腺腫との鑑別診断が重要となってくる．多くのprimaryempty sellaは無症状で経過するため，外科的治療の対象とならない．したがってどういう症例に手術適応があるか異論のあるところである。

A 34-year-old woman was admitted for the chief complaints of headache and blurred vision. She had bi-lateral papilledema and slight increase in CSF pressure (175mmH₂0) with normal visual acuity and field. Neurological and hormonal examination were normal except for over response of PRL and TSH to TRH test. The sella was enlarged and MRI and metrizamide CT demonstrated intrasellar CSF filling with remodeling of the pituitary gland. The patient was diagnosed as pri-mary empty sella syndrome associated with benign in-tracranial hypertension. The complaints did not sub-side for six months.
