もやもや病のEMS術後に発生した慢性硬膜下血腫

Chronic Subdural Hematoma developing after EMS for Movamova Disease

I．はじめに

　もやもや病の外科医的治療は，中硬膜動脈を含んだ硬膜片の脳表留置術，cervicalperivascular sympathectomy, superiorcervicai ganglionectomyなどがあるが，最近ではSTA-MCA吻合術，特に小児例においてはencephalo-myo-synangio-sis（EMS）^1）が試みられ，良好な手術成績が得られている．われわれは，小児もやもや病4例に対し両側EMSを行い，術後の脳血管写および臨床経過において，ほぼ満足すべき結果を得ているが，1例（2歳6ヵ月女子）に，術後，慢性硬膜下血腫の発生をみたものがある．文献的には中川ら^3）が同様の症例（5歳男子）を報告しているにとどまるが，これら2例とも年齢，頭部外傷の有無，および術後2ヵ月以内の発生，血腫発生頻度などの点から，開頭術そのものよりは，EMSが慢性硬膜下血腫発生に強く関与していることが疑われた．

　EMS血行路再建術の問題点および慢性硬膜下血腫発生の原因を考察するにあたり，われわれの経験した慢性硬膜下血腫合併例は興味ある症例と考えられたので，若干の文献的考察を加えてここに報告する．

A case of chronic subdural hematoma following EMS(Encephalo-Myo-Synangiosis) for moyamoya disease waspresented. Two and a half year-old girl was admitted toour division because of right hemiparesis. Left carotidangiography revealed a very narrow left carotid bifurcation and small moyamoya vessels. EMS was performed on theleft side and a right hemiparesis was improved. But shedeveloped headache and vomiting one month after theoperation. CT scan and angiogram showed a chronicsubdural hematoma on the left side. A hematoma weighingapproximately 30 g was evacuated.
