I．はじめに

　慢性硬膜下血腫の臨床症状としては，頭蓋内圧亢進症状，片麻痺，精神症状の3症状がよく知られている．このうち，はじめの2症状は頭蓋内の占拠性病変としての血腫に起因する症候として理解しやすいが，残りの精神症状に関してはまだ不明な点が多い．一般的にこの疾患は外科的治療によってきわめて良い結果が得られる疾患とされているが，ときに精神症状を残し，術後経過が予想に反して複雑になる場合がある．そこでわれわれは当院にて外科的治療を行った成人慢性硬膜下血腫32例について，①術前，術後にみられた精神症状の内容と特徴はどのようなものか．②術後精神症状を呈した症例に共通する因子としては何があるか．この2点につき検討を加え，さらに術後の精神症状発現に対する対策について推察を試みたので報告する．

Thirty-two cases with chronic subdural hematomain adults were analyzed from two standpoints; one isthe characteristics of pre- and post-operative psychiat-ric symptoms, the other is the common factors in pa-tients with post-operative psychiatric symptoms.1) The average age of subjects was 55.4 years, and psychiatric symptoms were noticed in 18 cases (56 %).

2) Main pre-operative psychiatric symptoms were dis- orders of intellectual capacity including memory and orientation disturbance and were able to be classi- fied into amnestic syndrome and dementia according to DSM-3.
