後頭蓋窩硬膜動静脈奇形の3症例—特に外科的治療について

Surgical Treatment for Three Cases of Dural Arteriovenous Malformation in the Posterior Fossa and Its Problems

I．はじめに

　後頭蓋窩の硬膜や天幕に発生する硬膜動静脈奇形（以下dural AVMと略す）は1968年Newtonら^13）が16症例をdural arteriovenous malfornlation in the pos-terior fossaとして発表して以来，多数の類似した症例が報告されている．さらにCTの出現，脳血管撮影法の進歩，手術所見や病理組織学的所見の集積により，本疾患の病態は次第に解明されつつあると思われる^{5-9,11,14，15,17,19）}．しかし，その治療方法は症例の病態により種々試みられているが確立されたものはなく，今なお試行錯誤の状態といえよう^16）．

　今回，著者らはNewtonらのいう後頭蓋窩duralAVMの3例（うち1例は脳動静脈奇形〔以下pialAVMと略す〕の併存したmixed AVM）を経験したので症例報告とともに主に治療方法について考察を加え報告する．

The authors reported 3 cases of dural arterio-venous malformation (AVM) in the posterior fossa and dis-cussed mainly on surgical treatment.

Case 1: 53-year-old man was admitted to our clinic with complaints of amnesia, visual impairment, dysarthria and gait disturbance. Neurological exami-nation on admission disclosed slight disturbance of consciousness, bilateral papilledema, horizontal nysta-gmus, dysarthria and ataxic gait. Contrast CT showed a presence of vermiform irregular high density with enlarged ventricle, indicating phlebectasia in the brainstem and the cerebellum.
