Long delayed post-traumatic brain abscess with unusual long latency over 15 years has been in-frequently reported in the literatures.

We have presented two cases of long delayed brain abscess and subdural abscess. These lesions developed 25 years after head injury in one case, and 19 years after in another case. These two cases were treated surgically with good results.

The pathogenesis of this lesion is not yet well known, but it was thought that adequete debride-ment and antibiotic therapy for the initial head injury will help to prevent this lesion.

The diagnosis and treatment of this disease were also discussed.
