はじめに

　従来，頭部外傷に引き続いて起る急性脳膿瘍および硬膜下膿瘍については秤々論議されてはいるが，初期外傷治癒後一定期間を経てから発症する外傷性遅発性脳膿瘍および硬膜下膿瘍についてはその報告が比較的少ない。なかでも外傷後10数年以上の経過で発症するものは稀であり，これらの診療に際しては外傷の既往をも忘れがちなためその診断にも問題が多い。われわれは最近頭部外傷後25年および19年という長期の無症状期をおいて発症した脳膿瘍および硬膜下膿瘍の2例を経験し，これを全治せしめ得たので報告し，併せてこれらの成因，治療，予防等について考察を加える。

Long delayed post-traumatic brain abscess with unusual long latency over 15 years has been in-frequently reported in the literatures.

We have presented two cases of long delayed brain abscess and subdural abscess. These lesions developed 25 years after head injury in one case, and 19 years after in another case. These two cases were treated surgically with good results.

The pathogenesis of this lesion is not yet well known, but it was thought that adequete debride-ment and antibiotic therapy for the initial head injury will help to prevent this lesion.

The diagnosis and treatment of this disease were also discussed.
