妊娠後期に破裂した前下小脳動脈動脈瘤

Ruptured Aneurysm of the Anterior Inferior Cerebellar Artery during Pregnancy

Ⅰ．はじめに

　頭蓋内動脈瘤の冶療についての最近の一般的考えは，統計的にも，経験的にも，直達手術が常識化され，1964年Adamsらによって報告された動脈瘤手術へのmicroscopeの導入は，その手術成績に画期的飛躍をもたらしつつある^12）．一方，後頭蓋窩動脈瘤はテント上のものに比し，頻度は少なく，Poolら^11）によれば約15％を占めるにすぎず，外科的療法なしには80％の死亡率を示すといわれる．しかしながらこれらのものに対する直達手術は，種々の点から困難なことが多い．部位的には，椎骨動脈，後下小脳動脈および脳底動脈と後大脳動脈分岐部に多く，それ以外のところには比較的稀である．なかでも前下小脳動脈瘤は稀である．われわれは，短時日の間に3度の出血発作をくりかえし，内水頭症を合併した妊婦の前下小脳動脈瘤を経験した．帝王切開を施行し，1か月後後頭下開頭で顕微鏡下に，左小脳橋角部に位置する動脈瘤にtrappingをおこない，患者は，神経症状を残すことなく治癒した．そこで，本症例にもとづき，妊娠後期に破裂した脳動脈瘤の治療をもう一度みなおし，また前下小脳動脈瘤手術について文献的に調査しその顕微鏡手術による治療の有効性について報告する．

Rupture of an aneurysm during pregnancy is most common in the third trimester and has several problems regarding its treatment. We experienced a patient with an aneurysm of the anterior inferior cerebellar artery (AICA) which ruptured during pregnancy and was successfully trapped under the operating microscope.

Case report: A 26-year-old woman (gravida ii, primipara) suddenly developed severe headache (suboccipital pain) in her 24 weeks of pregnancy on March 27, 1972. The next day, she became amnestic and lumbar puncture showed blooc'y CSF with initial pressure of 250 mm H2O.
