中毒性疾患とミオクローヌス

Myoclonus due to intoxications

はじめに

　ミオクローヌスを呈する中毒性疾患には各種あるが，多くの報告は単にミオクローヌスを認めたという記載にとどまり，その詳しい臨床症状や，筋電図，脳波など電気生理学的所見について述べたものはほとんどない。またミオクローヌスであっても単に「筋痙攣」とのみ記されているものもある。本稿ではミオクローヌスを呈する中毒性疾患についてその報告例を中心に述べ，さらに発症機序についても触れたい。

Abstract

Myoclonus in various intoxications were revie-wed and their etiologies were discussed. The causes of the intoxications include : tricyclic antidepressants, metrizamide, methyl bromide, chlorambucil, penicillin, bufulomedil, bromval-erylurea, enflurane, composition C-4, lithium, bismuth, lead, spinal anesthetics, L-dopa and other anti-parkinsonian drugs, MAO-inhibitors, 1-tryptophan, methysergide, valproate and others. In some cases myoclonus was due to anoxic encephalopathy which occurred during comatose state, but not to the toxic effect of the drug.
