I．Introduction

　痛みの感覚が末梢からAδおよびC線維によって脊髄視床路を経て視床に伝達されることは今や知られた事実であるが，視床におけるその投射野，organizationについてはいまだに不明の点も多く，いろいろな点で従来かなりの混乱が見られたように思える。しかし最近のこの方面の研究も著しく進歩して多くのことが明らかにされつつある。そこでここでは，これらの最近の知見をまとめたうえで，視床のレベルにおける痛覚の問題を考察してみたい。人間の痛みの問題を念頭に置いたために，主として人間を含めた霊長類での研究を取り上げ，それ以外の動物に関する仕事は必要最小限にとどめた。とくに解剖学的には種による違いが大きいと見られるからである^36）。視床を扱う場合，核の分類，命名などが問題になるが，原則としてOlzewski（サル）^41）とHassler（人間）^27）の分類に従って両者の関係は表1に示した。

Abstract

Recent anatomical and physiological understandings of thalamic organization of the spinothalamic pathway in monkeys and humans have been reviewed. Because of some technical and ethical problems, most of the experimental studies are on the so-called neospinothalamic (lateral spinothalamic) pathway that projects directly from the spinal cord to the thalamus and the studies on the paleo-spinothalamic (spinoreticulothalamic) pathway that projects indirectly via many relay stations in the brain stem are still few. The former projects mainly to the VPL and its surrounding structures such as VPLo (Vim), Pom and CL.
