Neuropsychological aspects of pure alexia syndrome

Ⅰ．純粋失読症候群の定義

　ここにとりあげるのは，純粋失読という現象ではなく，"純粋語盲"，"純粋失読"，"失書のない失読"または"失認性失読"などという名称で報告されているものの中で，左後頭葉内側部に限局性病変を有するものに生ずる1つの症候群についてである^1〜5）。したがって現象としては同様の失読症状を呈しながらもより広汎な病変を有するものや，これと異なった部分の病変を有するものは含まない。これはきわめて恣意的な定義であるが，従来の本症候群の病態に関する諸家の意見の相違は時として病変の性状や広がりに基づくものである可能性もあるため，あえてこのような定義を必要とするのである。また逆に，本症候群の意味するものは，単に純粋失読という現象のみにとどまらず，その他の高等機能異常をも含んでいる。visual-speech disconnexion^6〜8）という名称で呼ばれるものは，これとほぼ同じであるが，後述のごとく，本症候群には，必ずしもvisual speech disconnexionによると思われぬ現象も含まれるため，ここでは，左後頭葉内側部に限局性病変を有する患者の呈する神経機能の異常の全体像に対して，便宜上"純粋失読症候群"という名称を用いることとした。

　Neuropsychological studies were carried out on a patient with pure alexia syndrome caused by cerebral infarct due to left posterior cerebral artcry occlusion.

　Pure alexia syndrome is defined as a symptom complex resulting from a focal lesion of the left medial occipital lobe. The cardinal neuropsychological findings are alexia without agraphia, disturbance of operation of colors and object naming disturbance.
