The patient, 49 years old male, complainedof paralysis of the extremities with sensoryimpairment, slurring speech, dysphagia anddyspnoea. On thirty-fourth day after onsetof his illness, he lapsed into coma and diedof respiratory paralysis. At autopsy, in themedulla oblongata and spinal cord, manywedge-shaped demyelinated lesions which w-ere adjacent to mcningeal fluid were observ-ed. In the demyelination, proliferation ofmicroglia cells, appearence of macrophagescontaining myelin debris or fat granule cellsand destruction of axis cylinders were obser-ved. It was assumed that, although this casehad no demyelnation in the cerebrum andoptic nerves, it belonged to the category ofacute multiple sclerosis.
