筋無力症候群における神経筋接合部の微細構造

Ultrastructural changes of the neuromuscular junction in the myasthenic syndrome

はじめに

　神経筋接合部（以下NMJと略す）は，中枢神経のシナップスに比し，構造が単純であり，取り出して研究しやすいことから，神経伝達機構の生理学的・形態学的研究^1）に，近年盛んに用いられつつある。それとともに，重症筋無力症などの疾患について，その生理学的変化に対するNMJの形態学的裏づけを求めんとする研究^2）も試みられるようになった。一方，近年，末梢神経が単に筋肉の収縮・弛緩という運動機能に対する作用のほかにも，種々の影響を及ぼしている^3）ことがわかり，その面からもNMJの研究がなされるようになった。しかしながら，NMJの生理学的所見と形態学的所見を対応させることはなかなか困難である。

　本論文では，著者がNMJを電顕的に観察するために用いている方法と，今までに経験した重症筋無力症15例，Eaton-Lambert症候群7例の筋生検をもとに，それら疾患におけるNMJの微細構造について述べる。

The chance for the electron microscopic study of the motor end-plates was limited in the routine biopsy specimen. But the end-plates were easily observed under an electron microscope after the previous procedure to select the end-plate zone. Each piece of biopsied muscle was cut longitudinally into two strips after fixing with 2.5% glutaraldehyde for one hour. One was kept in cold phosphate buffer, and the other was stained for motor end-plates.
