神経筋疾患における接合部の問題点

Questions on the role of motor endplate in the pathogenesis of neuromuscular disorders

I．はじめに

　神経筋疾患（neuromuscular diseases）には，原因不明のものが少なくないが，なかには，実際の障害の場（site of the lesion）自体がはっきりしていないものもある。これは筋肉は，それ自身が侵された場合ばかりでなく，神経支配が侵されたことによっても変化を示すという，特別な条件下にあるために生じる問題で，たとえば進行性筋ジストロフィー症の成因に神経支配の異常が関与しているかどうかが今日でも議論の対象となるようなことも，このような事情がもとで起きてくるのである。神経筋接合部（終板）に異常があるかないかは，そういった意味で，ひとつの疾患が神経側から発生したか，筋側から発生したかを見定める資料のひとつとして，きわめて重大な意義を有することになるが，この場合にも，その異常が筋変化の結果的産物かも知れないという危惧が生じるために，常に二通りの解釈が成立してしまうことになる。このような危惧を打破する方法には，ひとつは時間的関係の確認で，筋に変化のないうちに接合部に変化を確かめれば，これが二次性であるとは考えられなくなることはいうまでもない。

The role of motor endplate in the pathogenesis of neuromuscular disorders has not been clearly understood. Morphological or functional abnor-malities of the plate have been recognized in various diseases, always a question has been raised whether or not such changes are secondary to the change of the muscle fiber itself. To answer the question, it is important to confirm the order of the appearance of the changes, i.e., which component shows the change first and which next.
