Ataxia telangiectasia, neuropathological investigation on 6 autopsy cases

Ⅰ．緒言

　免疫学の急速な進歩に伴い，ヒトの多くの免疫不全症候群^20）や免疫病^26）が知られてきた中で原発性免疫不全症候群^20）の一型として，近年とくに注目されてきた疾患にataxia telangiectasiaがある。この疾患は乳幼児期より発現する進行性小脳性運動失調症，球結膜や皮膚の末梢血管拡張症および呼吸器などの易感染性を主症状とする症候群であり，Syllaba ＆ Henner（1926）により初めてその臨床像が記載された^2,9,19,31）。その後Louis-Bar（1941）^22）により母斑症の一型とみなされた報告以来，Louis-Bar症候群としてポピュラーになった。1958年，Boder ＆ Sedgwick^4,5）の提唱により本症はataxia telangiectasia（以下A-T）と命名され，同年，本症の易感染性がCenterwallら^7）により初めて注目された。その後A-Tに胸腺リンパ系の低形成がみられ細胞性および体液性免疫不全が証明されること，さらに悪性腫瘍が高頻度に合併すること，および自己免疫現象^2）や老化性変化^19）が認められることなど次々に新しい事実が指摘されるに至った。

　Histopathological study on 6 autopsy cases of ataxia telangiectasia was carried out. The characteristic changes in nervous system and widespread abnormalities in the other organs were described.

　Pathogenetic considerations were performed with special reference to the cerebellar cortical atrophy, susceptibility to infection, as well as high incidence of malignancy in ataxia telangiectasia.

　It was pointed out that the nucleomegalic changes appear broadly in many kinds of cells derived every germ layer and seemingly in discontinuous replicator cells.
