急性播種性脳脊髄炎の2例

Two Autopsied Cases of Acute Disseminated Encephalomyelitis

Ⅰ．まえがき

　近年，ようやく多発硬化症をはじめとする脱髄性脳脊髄炎群の分類がなされ^1），わが国でもかなりの症例数が出揃い注目をあびるに至つた^17〜19）。しかし，依然として1920から30年代にわたり激しく論議^{2,3,8,13,14）}された急性多発硬化症（以下acute MSと略す）と静脈周囲性脳脊髄炎（以下PVEMと略す）の病理像をとる急性播種性脳脊髄炎（以下ADEMと略す）との間の異同の問題，さらにこれら疾患群の病因に関する問題など，今日なお結論が得られていない。著者らは，きわめて急性の経過をとり，それぞれacute MS，PVEMの形をとつた臨床上ADEMと診断された2例を検索する機会を得たので，上記の重要な課題の解明にとつて一つの手がかりになり得るものと考えて以下に報告する。

　Two autopsied cases of acute disseminated encephalomyelitis, that expired in the most rapid progress, were pathologically examined.

　The first case, 37 year-old male, complained of urinary retention and disturbed gait suddenly, developed fever and disturbance of consciousness subsequently, and died on the 8 th day after the onset. Neuropathologically, numerous sharply defined demyelinated lesions with inflammatory cell infiltration and slightly macroglial reaction are disseminated in the spinal cord, pons, midbrain, cerebellar and cerebral white matter.
