Behçet氏症候群における慢性播種性脳脊髄炎について—2剖検例より所謂Neuro-Behçet症候群の神経病理学への一寄与

Chronic Disseminated Encephalomyelitis in Behçet's Disease: A Contribution to the Pathology of so-called"Neuro-Behçet's Syndrome"

I．まえがき

　Behçet氏症候群は，1937年H．Bchçet^5）が再発を繰り返す口腔粘膜および陰部の潰瘍とブドウ膜炎を3主徴とする症候群を記載したのが最初である。その後，本症候群では3主徴のほかにしばしば結節性紅斑と皮膚の化膿性丘疹などを合併し，さらに精神神経症状の合併する（すなわち所謂Neuro-Behçet症候群）ことが明らかになつた^{6,12,19,41）}。このNcuro-Bchçet症候群（以下N-B症候群と略す）の臨床所見に関しては，これまでかなり多くの報告^{27,32,41,47,48）}があるが，剖検例の報告（表1）はなお未だ散発的であり，N-B症候群の神経病理学はなお未だ確立されていない現状にあるといつてよい。

　著者らは，それぞれ4年および6年余^43,44）にわたり脳幹症状を寛解と再燃とを繰りかえして，臨床的には多発硬化症に似た経過を呈したN-B症候群の2剖検例を検索した。この2例は，組織学的に髄鞘がかなり撰択的に障害されている慢性播種性脳脊髄炎の像が特徴的であり，一方では将来のN-B症候群に関する神経病理学の確立に寄与すると共に，他方，脱髄性脳脊髄炎の発生機序の問題に関連して多くの示唆が得られると考え，以下に2症例の報告と考察を加えたい。

　Two autopsy cases of Neuro-Behçet's disease were clinico-pathologically examined. The lesions were mainly localized in the brain stem and spinal cord. Histologically, both unsharply defined and perivascular demyelinated foci, restricted to the white matter, were confluentially disseminated. Myelin sheaths were markedly destroyed, and neuroaxons were, more or less, degenerated, while neuronal processes and axonal fibers regenerate vigorously.

　Authors suggested that some of Neuro-Behçet's diseases revealed pathoanatomically the picture of chronic disseminated encephalomyelitis.
