糖尿病患者の神経系障害に関する研究

A Study of the Disorder of the Nervous System in Diabetes Mellitus

I．緒言

　糖尿病患者における下肢の疼痛，パレステジー，並びに腰痛，坐骨神経痛の記載は，糖尿病自体が未だ極めてまれにしか観察されていなかつた18世紀末に，自らの臨床体験から糖尿病は胃の病気であると考えその治療法を工夫した英国軍医John Rolloの著書^66）にすでに見られる^30）40）。糖尿の本態がブドウ糖である事が判明し尿糖検査法が考案せられて糖尿病の診断が容易になつた19世紀前半には，糖尿病患者の神経症状の記載も幾つが散見せられるようである^30），しかしながら当時は未だ糖尿病の本態自体がなお暗中模索の時期にあり，肝に糖原の存在を実証したClaud Bernardが翌1849年家兎の第四脳室底の一定部位の穿刺により実験的糖尿を起こす事に成功し，糖尿病は肝における異化作用促進を起こす神経系疾患であるという考えを発表^2）してからは糖尿病の神経因説が有力視せられ，糖尿病の臨床または剖検例における神経症状ないし神経系障害はすべて一次的なものでそれを原因として糖尿が起こつているのであろうといつた観方しかなされていなかつたようである^5）。これに対して1864年Marchal de Calvi^7）は臨床土糖尿病の経過中に急に一過性の麻痺を呈し剖検上脳軟化を認めたが第四脳室底に何等異常を発見し得なかつた例等を挙げて，糖尿病患者の神経系障害には二次的と見なすべきものがある事を指摘した^5）。

The figures for the frequencies of the variousmanifestations with a circumspect scrutiny ofthe oblect mass have shown that the frequencyof the disorder in Japan does not greatly differfrom those in America or in the European coun-tries.

The most frequently observed four manifesta-tions, viz. the diminution or absence of the deepreflexes. the disturbance of the vibratory sensa-tioll, the pain, and the paresthesia dysesthesia, have been shown to have different ways of cor-relation to the clinical indices of the five nlainsupposed etiological factors, viz. the age, thelength of the duration of diabetes, the fastingblood suger leve1, the grade of diabetic retino-pathy or albuminuria, and the grade of Keith-Wagener in a fundoscopic examination or ofhypertension, and thus it has been demonstratedthat there are some distinct etiological differencesamong the various manifestations of this disor-der.
