Alexander disease is a rare leukodystrophy that usually affects infants, and is characterized by the progressive failure of myelination in the CNS, and by massive accumulation of Rosenthal fibers in astrocytes. Rosenthal fibers are cytoplasmic inclusions that contain abnormal aggregations of GFAP, in association with the small heat-shock proteins, αB-crystallin and hsp27. Rosenthal fibers are also seen in other pathologic conditions such as chronic reactive gliosis and pilocytic astrocytoma, but are an especially prominent feature of Alexander disease.
