Majority of cerebral infarctions are attributable to obstruction of the brain arteries by blood clot. Antithrombotic therapy is, therefore, the most essential treatment of cerebral infarction, which includes antiplatelet, anticoagulant and thrombolytic therapies.

In patients with acute ischemic stroke, aspirin has a modest effect in improving long-term outcome, and the thromboxane A2 synthetase inhibitor ozagrel is preferentially used for this purpose in Japan.
