心原性脳塞栓症の診断

Diagnosis of cardioembolic stroke.

脳梗塞ではCTやMRIによる病巣の検出のみでなく，血管と心臓の評価を行い，的確な臨床病型と発症機序の診断を行う。血管の評価は神経超音波検査，CT血管造影，MRA，脳血管造影などで可能であるが，頸部血管超音波検査や頭蓋内カラードプラ検査などの神経超音波検査では短時間に有用な情報を得ることができる。さらに心電図や心エコーによる心臓の評価が必要である。経食道心エコーは，経胸壁心エコーに比べ，虚血性脳血管障害の重要な原因である心原性脳塞栓症，卵円孔開存による奇異性脳塞栓症，大動脈のアテロームによる大動脈原性脳塞栓症の診断に有用である。卵円孔開存に伴う奇異性脳塞栓症は，若年者あるいは原因不明の脳梗塞の原因として重要である。奇異性脳塞栓症は，①動脈側に塞栓源がない，②卵円孔開存，心房中隔欠損症，肺動静脈瘻などの潜在的な右左シャントが存在する，③静脈血栓症もしくは肺塞栓症が合併する，④肺高血圧，一過性の咳，Valsalva負荷などによる右房圧の上昇がみられる，などにより診断される。Valsalva負荷コントラスト経食道心エコーや経頭蓋カラードプラで心臓での右左シャントを診断する。原因不明の脳梗塞では年齢にかかわらず経食道心エコーで卵円孔開存，大動脈病変，心疾患の有無を検索する。

The first testing step in acute stroke is an attempt to image the injured site by computed tomography or magnetic resonance imaging. In ischemic stroke, the intracranial/extracranial arterial disease should be detected by neurosonologic study, computed tomographic angiography, magnetic resonance angiography, digital subtraction angiograpy, or conventional angiography. In experienced hands, duplex and transcranial color flow imaging (or transcranial Doppler) method provide useful information within minutes, adding to the assessment of acute stroke.
