脳虚血性病変急性期の血管内治療

Endovascular treatment of cerebral ischemic stroke in acute phase.

1980年代後半から急速に進歩した脳血管内治療の技術と機材は，脳血管へのアクセスを容易にし，脳血管障害の治療に次々と新しい局面を切り開いている。その中で脳血管の狭窄や閉塞による脳虚血性血管障害に対する脳血管内治療は，従来の治療法とまったく発想を異にし，閉塞血管への直接的なアプローチによる閉塞血管の再開通，脳虚血から脳梗塞への進展を阻止する急性期治療を可能にした。とりわけ急性期脳塞栓症に対する局所線溶療法はその高い開通率と劇的な症状の改善が得られる治療法として広く普及しつつある。ここでは，この局所線溶療法を中心に，脳虚血性血管障害の急性期血管内治療について，その適応症例，適応基準，治療法の実際，今後の展望などについて考察した。

Recent advances of the techniques and materials of the endovascular treatment make it possible to make a new approach to the treatment of cerebrovascular disease. Especially in the acute phase treatment of the cerebral ischemic disease, the local fibrinolysis for cerebral embolism can reopen the occluded vessel, prevent falling into cerebral infarction and the patient recover from the ischemic symptoms in selected cases. And the PTA for the progressing stroke is also accepted as an emergency treatment with minimal invasion technique.
