痛みの治療―求心路遮断痛を中心に

Pain surgery―With special reference to deafferentation pain.

はじめに

　求心路遮断痛（deafferentation pain）とは比較的新しい言葉である。しかし，これによって表わされる疼痛そのものは昔から存在しており，以前は中枢性疼痛という言葉がほぼ同じ意味で使われていた。これは末梢性疼痛に対する言葉である。求心路遮断痛にせよ，中枢性疼痛にせよ，その治療はきわめて困難で，多くの先人たちの努力にもかかわらず確立された治療法は今までなかったといっても過言ではない。しかし，近年Nashold^20）が幻肢痛に対して脊髄後根進入部（DREZ）破壊術を創始して以来，ある種の求心路遮断痛に対しては治療が可能になってきた。本稿では主としてDREZ破壊術につき筆者の経験を中心に述べるが，それ以外の現行治療法についても言及する予定である。

“Deafferentation”is originally a neurophysiological technical term representing the state of secondary sensory relay neurons in the central nervous system, which are isolated from the primary imput of the peripheral nerves. In the state of deafferentation, the neurons become hyperactive, which is called as “deafferentation hyperactivity”. The term “deafferentation pain”was first coined by Tasker. According to him, the deafferentation pain is “…… all painful states associated with neurological injury, except the neuralgic pain associated with neurological compression, which is an example of somatic pain”.
