　抄録：脊髄・神経根領域の上行路遮断術としては，posterior rhizotomy，selective posterior rhizotomy，DREZ-otomy（DREZ-lesion），commissural myelotomy，cordotomyなどを行ってきた．これらのうち，posterior rhizotomy，commissural myelotomy，cordotomyはすべて癌性疼痛が対象であった．現在では麻薬による除痛が行われるため適応は限られる．DREZ-otomy（Sindou）あるいはDREZ-lesion（Nashold）は中枢性疼痛あるいは慢性疼痛に一定の効果が認められている．頚髄神経根引き抜き損傷では60～70％の例で長期的に良好な結果が得られるが，脊髄円錐・馬尾損傷例では有効例は50％台と低下する．治療に抗する例では後角の吸引除去などの，よりradicalな術式を考慮する必要がある．
