はじめに

　三叉神経痛の治療として中枢に対する手術を行ったのはSjöqvist^11）が最初である。彼は延髄で三叉神経脊髄路の切断を行った（trigeminal tractotomy）。しかし，これは特発性三叉神経痛には有効だったが，ヘルペス後三叉神経痛には無効であったと彼自身述べている。この術式は一時は大いに行われたが，特発性三叉神経痛に対してはJannetta法をはじめよい術式が次々と開発されたので今では特発性三叉神経痛に対して中枢への手術をする人はほとんどいない。

　しかし，ヘルペス後三叉神経痛などの求心路遮断痛deafferentation painに対しては効果的な治療法がなかった。Hitchcock^3）は定位脳手術の技法で三叉神経核の凝固を行い，ヘルペス後三叉神経痛を消失せしめたと報告したが技術的に難しいのと，効果が一定しないために普及しなかった。

Surgical approach to the central nervous system for the treatment of essential trigeminal neuralgia was first tried by Sjöqvist, who interrupted the descending trigeminal spinal tract at the medulla. However, this method was not effective to the deafferentation pain such as post-herpetic trigeminal neuralgia (PHTN).

Nashold proposed the “Dorsal Root Entry Zone lesion” (DREZ-lesion) for the phantom limb pain after the brachial plexus avulsion injury. He applied this technique to the atypical facial pain including PHTN by making many lesions in the subnucleus caudalis. The results were reportedly very good.
