ALSにおける喉頭摘出術の意義

Total laryngectomy for misdeglutition in ALS patients.

はじめに

　筋萎縮性側索硬化症（以下ALS）は，1869年Charcotにより報告された，上位運動ニューロンと下位運動ニューロンが選択的に変性する進行性の疾患である^1）。罹病期間は2～4年^2,3）である。ALSの死因では呼吸困難肺炎などが多いとされるが^4），最近ではレスピレーターの利用を含む呼吸管理技術の進歩により，7年以上の長期例も報告されるようになった^4,5）。このためにALSの晩期においては，嚥下性肺炎の予防すなわち誤嚥の防止が重要な課題となってきている。当科においては，昭和58年以来ALS症例の誤嚥に対して積極的に外科的治療を施行しており，今回とくに単純喉頭全摘術の意義について述べる。

Amyotrophic lateral sclerosis (ALS) is a progressive mortor neuron disease reported by Charcot in 1869. The management of the dysphasia (misdeglutition) is most important for late stage of ALS. Recently various surgical procedures have been proposed to either eliminate or alleviate misdeglutition in ALS.

In this paper, I focused total laryngectomy among other various procedures. Dysphagia in ALS is a dynamic disorder of swallowing. Fluoroscopical and electromyographical examinations revealed marked disturbance of the oral stage.
