クロー氏喉頭摘出術に就ての感想

　緒論

　喉頭摘出の術式に就いては本邦に於ては2つの流れがあるように思われる．即ち笹木教授は其の著書より拜見すればグルツク氏法に従つておられるらしい．鳥居教授の手術書も主としてグルツク氏法を記述されている．之れに対し従来から小田前岡山医大教授は専らクロー氏法を推奨せられている．之に対し鳥居教授は本法が優秀な方法であるという批評を与えておられるが．唯上喉頭動脈の結紮が窮屈であるという点を指摘されている．いう迄も無くグルツク氏法は喉頭全面に広大な皮膚切開を施し，喉頭全面の諸筋を努めて切断して与う限り喉頭全景の露出を容易にし，然る後喉頭の摘出を行わんとするものであり，之に対しクロー氏法は専ら喉頭の摘出を主眼とし手術野の拡大を能う限り最少限に止めて其の効果を発揮しようとする．従つて理論的に考察して，もしもクロー氏の説く如き方法で充分喉頭摘出の目的が達せられるならば，グルツク氏の如く聊迂縁な方法を講じなくとも差し支えないのではないかとの感が起る。然し他方グルツク氏の方法は既に永く世の批判に堪えて来た方法であり，クロー氏の方法はいわば之に対する変法に過ぎないような感もあり，従つて私は手術に於て何れの方法を取る可きか聊か之に迷いを感ぜざるを得なかつた．依つて私は其の後私自身の見解を明確にする為，最近少しくクロー氏法に就いての臨床経験を重ねて見た．未だ充分多数の症例の経験を重ねたというわけではないが，とにかく現在の立場に於て私なりに1つの感想を得る事が出来るようになつたので其の事を茲に報告したい．

KASIIIWADO performed laryngectomies, accor-ding to the method described by Crowe during the period of past 3 years, on 7 cases laryngeal carciu-oma and lcas of canar of pyrform fossa. Three pa tients died and 4 are living, In one of the 3 cases who died the cause of death was vecuvrence of cancer at the amputated end of the trachea : in the other, metastasis to celvical lymph glands: and the third whs relegated to unsuccess ful resection of esophagus. The author believes that even it Glu-cke's operation had been performed instead of Cro-we's, the ultimats results of these cases would not have differed. As far as post-operative courses of the cases are concerned Crowes method seems to be by far superior when the two are compared. This fact may be explained in that't affores mini-mum injury to the soft tissues of the neck.
