筋・神経系疾患における在宅人工呼吸器看護15症例の経験から

The home care services for mechanical ventilation patients in community.

I．在宅医療における在宅人工呼吸器療法

　近年，患者の人権を保障する観点などから，在宅医療ニーズが高まり，検討が行なわれている。在宅医療の対象は，いわゆる寝たきり老人から在宅人工呼吸器療法など医療依存度の高い症例まで幅広いが，在宅医療システムを検討する過程としては，医療依存度が高い対象から検討を行なうことは課題を明確にできる利点を有する^1）。

　本来，在宅医療は患者や家族の意志により，医療環境として整備不十分な一般市民生活の場において医療を行なうものであり，医療機関内と異なるシステムを構築する必要性がある^2,3）。本稿では在宅医療システム課題のうち，医療課題を中心に検討することとする。

This report describes the findings of 15 home mechanical ventilation patients. 11 cases of them were having Amyotrophic Lateral Screrosis. They and their families eagerly desired to receive treatment mechanical ventilation at home. They required for it's operation, a suitable environment and medical prosessionals having the necessary knowledge and techniques such as physicians, nurses or paramedics.

In order to care for 15 home mechanical ventilation patients, we have cooperated with the health care practioners, public health nurses, home nurses, other health related manpowers and social welfare personnal in the required areas, and have organized them into a local support net work for each cases.
