神経疾患における喉頭摘出術の適応と効果

The indication and benefits of laryngectomy for patients with neurological disorders.

重篤な嚥下障害で誤嚥が頻回に生じ，かつ構音障害が強くて発話による意思表出がほとんど不可能な神経疾患例において，われわれが試みている喉頭摘出術（喉摘）の適応と効果について述べた。ALSは進行性の神経変性疾患であるが，中には喉摘により誤嚥の恐怖や誤嚥性肺炎から完全に解放され，かなり長期間経口のみで十分の栄養摂取が可能となるものがある。未だ症例数が少ないため断定的なことはいえないが，このような症例の背景としては，手術に踏み切るタイミングが早期であること，呼吸状態が安定していること，経口摂取に対する欲求が強いこと，以上3点が重要であろうと考えられた。

MSAでは，喉摘によって誤嚥と声帯麻痺による突然死を完全に防止し，さらに経口摂取できるようになる可能性がある。

We describe the indication and benefits of laryngectomy for patients with neurological disorders which cause se-vere dysphagia with frequent aspiration and such a severe dysarthria as for the patients to be unable to communi-cate with speech.

Although amyotrophic lateral sclerosis (ALS) is a rapidly progressive neurodegenerative disorder, we have ex-perienced several patients with ALS who, after laryngectomy, became not only completely free from fear of aspira-tion or aspiration pneumonia but also came to be able to obtain adequate nutrition orally again for a long time.
