膀胱真菌症を合併した陰茎壊疽

FOURNIER'S GANGRENE ASSOCIATED WITH FUNGUS BALL IN THE URINARY BLADDER : A CASE REPORT

　52歳，男性．数年来の糖尿病，および慢性アルコール中毒治療中，血尿を主訴に当院紹介された．経過中陰部は次第に発赤腫脹をきたし，突如壊死に陥った．陰茎壊疽の診断にてデブリドマン，洗浄，抗生剤の全身投与を行った．またCTにて膀胱内に葡萄の房様腫瘤あり，生検の結果，膀胱真菌球形成の診断にて抗真菌剤の膀胱内注入にて腫瘤は自壊し，血尿も消失した．

Necrotizing infection of penoscrotum (Fournier's gangrene), initially reported by Fournier in 1883, had become unusual with the recent progress of antibiotics, while fungal urinary tract infection is not so rare. We experienced a patient with these two disorders at the same time. A 52-year-old male complaining of macrohematuria was referred to our hospital, after he had gotten conservative therapy for diabetic coma. He had diabetes and chronic alcoholism for twelve years. On admission mild swelling in the perineum with indwelled Foley catheter was noted.
