膀胱全摘術後患者の生活調査

THE QUALITY OF LIFE IN PATIENTS WITH BLADDER CANCER AFTER RADICAL CYSTECTOMY

　当科で膀胱全摘術後1年以上の経過観察を行っている33例に対して，尿路変向法を1群：尿管皮膚瘻（13例），2群：回腸導管（6例），3群：外観上ストマを有する代用膀胱（5例），4群：外観上ストマを有しない代用膀胱（9例）に分類し，生活の質に関する調査を行った。全群で食欲・睡眠・入浴の復帰率は良好であった。代用膀胱群（3・4群）に尿の漏れが多かったためか，各群問に有意な生活の質の差はみられず，与えられた尿路変向法を受容していく傾向があった。筆者らは適応を熟慮して代用膀胱を作るのが望ましいが、術後合併症を少なくすることが重要であり，それが与えられた尿路変向法への受容，ひいては満足にもつながると考えられた。

A questionnaire survey was performed to assess the quality of life in 33 patients who were followed up after radical cystectomy more than one year at our hospital. The urinary diversions were classified into 4 groups; ureterocutanostomy in 13 patients, ileal conduit in 6, continent urinary reservoir with apparent stoma in 5, continent urinary reservoir without apparent stoma in 9. In all groups, appetite, sleep and bathing were well kept postoperatively.
