腎盂腫瘍の副睾丸・精索転移

METASTATIC TUMOR OF THE EPIDIDYMIS AND SPERMATIC CORD RESULTING FROM CARCINOMA OF THE RENAL PELVIS

　73歳，男性．左腎盂腫瘍の手術後約3か月の経過観察中に左副睾丸部に母指頭大の腫瘤を触れるようになった．手術を行ったところ，左精索にも小豆大の腫瘤を認めた，病理組織検査で副睾丸，精索の腫瘤とも腎盂移行上皮癌の転移であると確認された．腎盂腫瘍の精索，副睾丸転移は報告がなく，本邦第1例目である．著者らは転移性副睾丸，精索腫瘍の本邦報告例62例について集計し考察を加えた．

A 73-year-old man. who had undergone left nephroureterectomv due to a renal pelvic tumor 3 months previously. was found to have a 3 x 2 cm indurated mass in the left epididymis. As the lesion did not improved with antibiotic therapy. metastasis was suspected and radical orchiectomy was performed. A small beansized nodule was also found in the spermatic cord during the operation. Histological examination revealed the epididymal tumor and the nodule of the spermatic cord to be metastatic transitional cell carcinoma resulting from the renal pelvic tumor.
