腹壁原発と考えられた悪性線維性組織球腫

Malignant fibrous histiocytoma of the abdominal wall:A case report

　症例は63歳，男性。2000年5月臍左下部に6cm大の腫瘤を指摘され，尿膜管腫瘍を疑い同年7月19日，尿膜管摘除術に準じて手術を行った。病理組織学的にはMFHであった。術後8か月目に骨盤腔内への再発を認め，術後15か月目に死亡した。自験例を含め，腹壁に発生したMFH6例を集計した。腫瘤摘除術を試みた6例中4例は再発が報告されており，初回切除時に広範囲切除術を行うことが重要であると考えられた。

A 63-year-old man was found to have a left abdominal mass. MRI showed that a tumor, which was located on the abdominal wall in upper region of the urinary blad-der, was heterogeneous and 6 cm in size. An urachal tumor was suspected and surgery was performed. Although we found that the tumor was completely sepa-rated from the urinary bladder during the operation, we remove the tumor with some part of the abdominal wall, bladder dome, navel, and urachal ligaments. The patho-logical examination revealed that the tumor was malig-nant fibrous histiocytoma (MFH), not urachal tumor.
