腎摘除術後10年を経て小脳転移した腎細胞癌

Cerebellar metastasis from renal cell carcinoma occurred ten years after radical nephrectomy

　腎摘後10年を経て小脳へ孤立性に転移した症例を経験したので報告する。症例は75歳，男性，歩行障害を主訴に受診した。MRIにて小脳半球に円形で均一に造影される2cmの腫瘍が認められ，腎癌小脳転移の診断で腫瘍摘出術が行われた。組織学的にはclear cellから成ることから，腎癌の脳転移と診断した。術後，症状は消失し，QOLが得られた。今回の症例では小脳のみに転移が認められたことより，肺動脈循環を経由しないバトソン静脈叢を経由して起こったものと考えられた。

We report a case of a solitary cerebellar metastasis from a renal cell carcinoma (RCC) 10 years after radical nephrectomy. A 75 year-old man was admitted to our hospital with the main complaint of gait disturbance. Magnetic resonance imaging (MRI) revealed a round mass (2cm) in the cerebellar hemisphere, homogene-ously enhanced on post contrast images. With a clinical diagnosis of cerebellar metastasis from the RCC, a total removal of the cerebellar tumor was performed. Histo-logical, diagnosis was clear cell carcinoma, suggesting metastasis of the RCC.
