移行上皮癌と重複した転移性膀胱印環細胞癌

A case of simultaneous transitional cell cancer of the urinary bladder and metastatic signet-ring cell gastric carcinoma

　症例は68歳，女性。右上腹部痛精査の内視鏡検査にて胃に隆起性病変を認め，生検にて印環細胞癌が確認された。その後，肉眼的血尿が出現したため当科を受診した。膀胱鏡検査で乳頭状腫瘍を認め，経尿道的膀胱腫瘍切除術を施行した。組織学的に表在性移行上皮癌とその粘膜下に印環細胞癌を認め，原発性膀胱癌と転移性膀胱印環細胞癌の重複と診断した。5-FU内服療法にて経過を観察していたが，術後5か月後に死亡した。

A 68-year-old female underwent gastroscpy and biopsy because of upper abdominal pain, which was diagnosed as signetring cell carcinoma of the stomach. She visited us complaining of grosshematuria. Cystoscopic examina-tion revealed a papillary sessile tumor and we per formed TUR-BT. Histological finding revealed superfi cial transitional cell carcinoma and adenocarcinoma with signet-ring cells in submucosa of the urinary bladder,and there was no histological continuity between them.We made a diagnosis of double cancer of the urinary bladder, medicated her with 5-FU. She died 5 months after the operation.
