膀胱原発平滑筋肉腫の1例

Primary leiomyosarcoma of the bladder:A case report

　症例は脊髄損傷で自己導尿中の27歳男性。膀胱タンポナーデで緊急入院となる。膀胱鏡で膀胱後壁に3cm大の粘液様広基性腫瘍を認め可及的に切除。膀胱平滑筋肉腫の診断を得た。膀胱外病変の所見はなく，骨盤リンパ節郭清および膀胱部分切除術を施行。摘除標本に残存腫瘍を認めなかつた。患者は術後1年6か月経過した現在，再発・転移の所見はなく生存中である。膀胱平滑筋肉腫の予後規定因子につき若干の文献的考察を加える。

A 27-year-old male with spinal cord injury, whose blad-der drainage had been managed with intermittent cathe-terization, was admitted to hospital because of bladder tamponade. Cystospopy revealed a retrotrigonal myxoid tumor of 3 cm in diameter, which was endoscopically resected deeply as possible.A diagnosis of leiomyosar- coma was made. He underwent pelvic lymph nodes dis- section and partial cystectomy. Histopathologic examina- tion revealed no residual tumor in the resected speci- mens. He is free of the disease 1.5-year after surgery. Prognosticators of leiomyosarcoma of the bladder are briefly discussed.
