扁平上皮癌化した精巣奇形腫

Testicular Teratoma with Malignant Transformation:A Case Report

　33歳，男性。無痛性右側陰嚢腫脹を認め来院した。超音波検査上，陰嚢内血腫が疑われた。しかし，腫瘍マーカー高値のため精巣腫瘍の存在が示唆され，高位精巣摘除術が施行された。摘出標本の病理組織所見は，未熟奇形腫，胎児性癌および卵黄嚢腫瘍より構成される複合組織型の精巣腫瘍であり，奇形腫の一部に扁平上皮癌への悪性化が認められた。

A 33-year-old male patient presented with right scrotal enlargement. Ultrasonic study disclosed intrascrotal hemor-rhage, probably resulted from minor testicular injury received 3 months before. Since the serum levels of AFP and HCG-β were elevated, orchiectomy was carried out. Histopathologically, the tumor was composed of immature teratoma containing cartilages and smooth muscles, em-bryonal carcinoma and yolk sac elements. Malignantly transformed squamous cell components were also seen in part of teratoma.
